Home Education Enrollment Process
For the Upcoming School Year by August 1: Home Education Annual Enrollment Process
Please submit the following information to Mrs. Meghan Lloyd, Supervisor of Teaching, Learning, and Technology to the address listed above: If NEW to Whitehall-Coplay School District, please register your student using the Centralized Enrollment link.
- Notarized affidavit or Unsworn Affidavit
- Notarized Affidavit (Elementary Sample, Secondary Sample)
- Unsworn Affidavit (Elementary Sample, Affidavit - Secondary Sample)
- Outline of Educational Objectives by subject for each student
- A Physical Exam Form or a religious exemption letter
- Evidence of Child’s Immunizations or religious exemption letter
- Dental Record
- Academic records or transcripts from prior school (if not a WCSD student)
- Special Education Prior to starting Home Education (if applicable)
- Written notification of approval that the specific support needs of the student are addressed by the program. The approval must come from an evaluator who has one of the following qualifications:
- PA certified special education teacher
- A licensed clinical psychologist
- A certified school psychologist

We will notify the home education supervisor by mail or email indicating that their child has met the requirements for Home Education the first week in August. If your child is currently enrolled in a public or non-public school, do not remove him or her from that education placement until you have received the approval letter.
The deadline for Affidavits is August 1st of each school year.
