Vision screening has been completed for the year. A referral letter (which includes a doctorevaluation form on the back) was sent home for those students who need further evaluation by aneye care specialist. *Parents, please remember to send in the completed form when your child hasbeen to the specialist. The School Health Law of Pennsylvania requires that all 3rd grade students have a dental exam. Thisis exam is due back to the school nurse before the end of the school year. Our school dentist will becoming to Steckel on April 12th. *For those students who still need to have this exam, parents can send in a signed note to the school nurse requesting this free exam.
If your child needs to take any medication during the school day, the “Medication Dispensing Form” will need to be completed and signed by both a parent and doctor. A parent/guardian should give medication before or after school hours if at all possible. If a child must receive medication during school, the medicine must be sent in the original container labeled with the child’s name. Both prescription and over the counter meds (Tylenol, Motrin, etc.) must have a physician’s order to be given during the school day. No medication will be administered in school without a completed Medication Dispensing Form. Copies of this form are available from the school nurse. The form can also be downloaded by clicking on the link above in the "Health Forms" box. Please contact the school nurse with any questions.
The proposed changes to the school immunization regulations have passed and will become effective on August 1st 2011. Children in ALL GRADES will need 2 doses of Varicella (chickenpox) vaccine. Your child’s health record will be reviewed and a letter will be sent home if your child needs to have their 2nd varicella shot. If your child has been to the Dr. recently and has received this 2nd dose, please provide documentation to the school nurse (if you haven’t done so already). If you need help obtaining immunizations for your child, please contact Mrs. Dorner at 610-435-1521 Ext 5.
Parents of 3rd Grade Students
A dental exam is required by the state of Pennsylvania for all students in 3rd grade. If your child has recently seen the dentist and you have not returned the yellow “Private Dentist’s Report” dental form, please return the completed/signed exam form to the school nurse as soon as possible. Parents also have the option to choose a school dental exam for their child. This is a free exam for those students who return their signed choice form to the school nurse. This exam will be done in spring 2014. If you need to have another choice form, please call the nurse at 610-435-1521 #5.
