Health Emergency Form:
Please complete your child's Health Emergency form and return it to school as soon as possible. This is our onlyl way to contact you in the event of an emergency.
Physical/Dental Examinations:
Parents are encouraged to have their child's pediatrician or family physician and dentist complete physical and dental examinations. Parents may consent to have the school doctor and/or dentist examine their child. This examination is offered free of charge.
Medication Administraton:
Any child that requires medication during the school day must follow the district policy for medication administration. Medication must be brought in the original bottle. The medication will be kept in the health room. It will be the student's responsibility to go to the health room at the correct time and request the medication. In the absence of the school nurse, the principal or designee may give the medication.
All prescription medications must be brought to school by the parent/guardian.
For non-prescription medications, the "Non-prescription Medication Dispensing Form" must be completed by the parent and submitted to the school. For prescription medications, the "Prescription Medication Dispensing Form" must be completed. Both the parent and physician must sign this form.
Students are not permitted to carry medication during the school day. School policy allows students to carry asthma inhalers. In order to carry an asthma inhaler, the student must have the permission of his/her physician, parent, and the certified school nurse.
